Home	Downloading of Information Sheets

The information sheets are Microsoft Word documents, which are compressed in a file. Here are the instructions :

1. Download the sheet by simply clicking on its name.
   The downloading allows you to get the file directly onto your computer.
   
   
2. The file you get then is named the_sheet_name.exe. It is a self-extracting zip file for DOS (created with WinZip). Once the file is on your computer, it requires to be executed in order for the information sheet to be extracted.
   
   
3. The extracted file is a Microsoft Word document,which you will be able to edit in order to fill it in.
   
   
4. You may then send us back the completed information sheet :
